Job Description
Role Overview: The Community Support Worker provides person-centered care and support to service users in their own homes.
- Assist with personal care tasks, including bathing, dressing, and continence care.
- Support mobility using aids and adaptations as needed, ensuring dignity and privacy are maintained.
- Administer medications according to care plans and company policies.
- Help with meal preparation and hydration to meet dietary needs.
- Assist with light household tasks, appointments, and social activities to promote well-being.
- Foster an environment that encourages independence and emotional well-being.
- Observe and report changes in health to the Care Coordinator or Care Manager.
- Maintain accurate records and ensure clear communication among care teams.
- Participate in ongoing training to enhance skills and knowledge.
