About the role
Looking to make a meaningful impact on dementia and delirium care in hospital?
As a Dementia and Delirium Practitioner, you’ll be key in supporting patients in our Medical & Elderly Ward during their hospital stay, helping to create a safer, more supportive environment.
You’ll help raise awareness of dementia and delirium across the organisation, though appropriate assessments and education, whilst establishing and supporting the delivery of diversional therapy, becoming a crucial link between the hospital, community and third sector organisations, and making sure people receive consistent care across every setting.
